Terms & Conditions of Registration

Before proceeding with your registration, please read the following terms and conditions carefully:

 

All participants must register through the online registration form. Reservations for workshops and roundtables may be cancelled if full payment is not received in a timely manner, and the space will be released for sale to other delegates. Payment via the online payment system is strongly recommended to ensure your selections are confirmed.

 

Cancellations:

Notifications of cancellations must be made in writing and sent by email to the CLMC Congress Secretariat (info@clmc.ca) on or before May 31, 2015 in order for registration fees to be refunded. If you have been invoiced but not paid the fees, you are still liable to pay the cancellation charges below:

• Up to and including May 31, 2015: Registration fee will be refunded less $50 administration fee.

• After May 31, 2015: No refunds on registration fees will be made. Substitutions can be made.

 

Special Access and Food Requirements:

Please list essential special food requirements only. In general, choice of food is the delegate’s responsibility. Special dietary requirements can only be accommodated through advance registration.

 

Member Rates:

The discounted Member Registration Fees apply to current members of CAP-ACP, CSCC-SCCC, and APQ. All other delegates should register in the Non-member category.

3PA Program Registration includes: access to all scientific sessions in the PA Program schedule (Saturday, Sunday), lunch (Saturday), and Presidents’ Welcome Reception (Sunday). Not included but available for purchase at an additional fee: PA Section Dinner (Saturday).

4APQ Monday Only Registration includes: access to all symposia, lectures, SIG sessions, industry workshops and satellite symposia, APQ AGA and Wine & Cheese Reception, on Monday June 22. Only Monday may be attended at this rate – attendance on multiple days requires full registration. Not included: Scientific Workshops, Breakfast Roundtables, Presidents’ Welcome Reception, and Conference Banquet.
